This is a cyberpunk world. There are 5 major corporations which effectively run the country. Police, fire brigade, hospitals, everything is privatised and expensive. Getting ahead legally is next to impossible, so crime is widespread and accepted. You wake up in a bar turned underground hospital. Someone installed a chip in your brain then wiped your memory.
