He is a bit taller compared to other people, with pale white skin, pointed ears, sharp fangs, and dark blue eyes. He has blonde hair mostly hidden within a purple scarf. His bangs reach to the shoulders, and the left side is long enough to be tied with a bright blue band. The outer parts of his eyes are outlined by red marks and those below form the appearance of tears. He seems always to be smiling, but it's due to the purple mark on Kusuriuri’s upper lip. His attire consists of a light teal green kimono with psychedelic images at the ends of the sleeves and a dark red obi with golden swirl marks at the waist. Around his neck are several necklaces with small intricate jewels, including a gold amulet. He wears flowing black pants underneath that fall just below his knees and his lower legs are wrapped with bandages. Kusuriuri also wears a pair of black socks, his primary footwear being a pair of wooden geta sandals with two prongs. Kusuriuri is generally seen carrying a large wooden box filled with medicines and sacred tools, including paper ofuda charms, scales that detect supernatural presence, and a secret collection of feudal pornography. The large box also contains a smaller personal box housing a strange and powerful sword used to kill Mononoke; the sword is enchanted such that to use it, the medicine seller must find the Katachi (physical form), Makoto (truth or origin), and Kotowari (reasoning behind an action) of a particular Mononoke he wishes to exorcise. Once the sword is unsheathed, it transforms into a mystical and powerful being that is more or less an inverted version of himself. This being wears a golden kimono with a bright red obi and dark purple pants. His hair is long, wild, and white, and his skin turns a dark tan. The being’s eyes are red with black sclera, and the simplistic red markings on Kusuriuri’s face become more detailed, enveloping the entire body of the being in golden color. He also wears large circular earrings with bracelets hanging from them.

Kusuriuri is, in the literal sense, a walking contradiction. He claims to be "uninteresting" to other people, yet his strange appearance makes him stand out from the crowd, making him easy to identify. He does not reveal or show much of his own history, but his profession involves uncovering the history of others to identify the mononoke he hunts. He remains calm and serene even in extreme situations. Kusuriuri is observant and analytical, careful when examining every detail of a broader scenario. His moods range from the usual impassive demeanor with which he is often seen, to slight amusement and irritation if provoked. In extreme cases, he can exert a moment of intense emotion, but that is rare and intermediate. It's also revealed he is a pervert, as he has several "marriage charms" and pornography books in his possession, and he gets embarrassed when the subject is mentioned. Kusuriuri is witty and somewhat sarcastic in response, and for those who generally get on his nerves or don’t believe in Mononoke, he resorts to subtly insulting them. However, those who accept the unknown and him are generally treated more kindly.
