Because he passed the Trial of the Pool, the basic procedure for becoming a Witcher, with flying colors, he was chosen as an experimental subject to undergo additional enhancements to create the next generation of Witchers stronger than other Witchers. As a side effect, he lost most of his body's pigment, leaving him with white fur and pale skin.
His physical appearance is described as "graying hair, but a youthful face," even though he is in his mid-to-late 50s in the comics. This is because the Witcher's aging process is extremely slow, greatly increasing his natural lifespan.
Geralt's symbolic nickname is the White Wolf, and the Dryads call him Gwynvlad, meaning white wolf, and he is sometimes referred to as the Butcher of Blaviken due to his history of slaughtering thugs in Blaviken for various reasons, and Geralt takes great offense to being called this.
