幻想郷は外の世界から隔絶された異世界。
結界によって守られた土地であり、外の世界で忘れ去られたもの、幻想となったものがこの地へ流れ着く。

科学技術は発達しておらず、魔法や妖術が日常的に使われる。
外の世界の常識は通用しない。ここでは幻想こそが現実。

博麗神社: 幻想郷の東端、外の世界との境界に位置する神社。博麗霊夢が巫女として管理している。参拝客は少なく、いつも静か。神社の裏手には森が広がり、境内には古びた賽銭箱と鳥居がある。霊夢は異変解決を生業とし、幻想郷の秩序を守る役割を担う。
紅魔館: 霧の湖のほとりに建つ西洋風の巨大な館。吸血鬼レミリア・スカーレットが主人として君臨し、妹のフランドール・スカーレット、メイド長の十六夜咲夜、魔女のパチュリー・ノーレッジ、門番の紅美鈴らが住む。館内は迷宮のように複雑で、大図書館や地下室などが存在する。昼間でも館内は薄暗く、吸血鬼が住みやすいように調整されている。
人里: 幻想郷で唯一、人間が集まって生活する集落。妖怪から身を守るため、結界と自警団が存在する。商店、酒場、寺子屋などがあり、日常生活が営まれている。外来人が最初に訪れることが多い場所。
魔法の森: 深い森で、毒キノコや魔法植物が自生する。霧雨魔理沙の家がこの森の奥にある。妖怪や妖精が多く棲み、迷い込むと出られなくなることも。
